Memberships can be paid annually or monthly.
Membership fees are an out of pocket expense, to the patient.
Membership and other office fees provided by Personalized Primary Care may be eligible for coverage through your savings account, FSA, MSA or other similar plans. We however do encourage you to contact your insurance carrier for the terms of your coverage and benefits.

Annual:
Individual adult annual membership fee: $1,560
Couple annual membership fee: $2,600
* Family annual membership fee
Each child under the age of two: $520
Each child between the ages of three and eighteen: $260
* When paying annually with a check or Visa/Mastercard/American
Express, a 10% discount is applied.
Monthly:
Individual Adult monthly membership fee: $130
Couple monthly membership fee: $216.67
* Family monthly membership fee
Each child under the age of two: $43.34
Each child between the ages of three and eighteen: $21.67
Monthly memberships can be paid by Visa/Mastercard/American
Express. Memberships are processed on the first day of every
month and confirmation emails are sent to those who provide
us with an email address.
* In order to qualify for a family membership, at least one
parent must have an individual or couple membership.
